Using ECG Guidance for central line catheter tip placement

The basic principle of the ECG method is that the position of the tip inside the venous system can be detected by using the catheter itself (or a guidewire inside the catheter) as an endovascular electrode, which replaces the “red” or “right shoulder” electrode of the standard surface ECG.

When the tip of the central venous catheter reaches the superior vena cava and advances towards the right atrium, the P-wave gains in amplitude. When the P-wave is at half of the QRS wave or at its highest amplitude, without being biphasic (the P-wave must show no negative feature which would indicate that the catheter is within the right atrium and must be slightly withdrawn), the catheter is in the correct position.

The waveforms are generated at the tip of the central venous catheter via the guidewire which are captured in real time through the combcard adaptor and displayed on your ECG monitor.
