Product: Halyard Health Closed Suction Kits with Flex Connector

We have been instructed by the manufacturer to issue an FSN following reports indicating that certain Flex Connectors supplied with the Closed Suction Kits may become loose or disconnected before use or during use.

Please find attached a copy of the FSN and the fax/email back form through this link. The FSN has been sent to all customers of the affected codes.
